Report an event that resulted in injury, damage, equipment failure, or another operational incident requiring documentation and follow-up.
Safety and Near Miss Reporting helps us identify trends, improve procedures, address potential hazards, and learn from real experiences across the organization. Many incidents are preceded by smaller warning signs or close calls that, when shared, can help prevent future problems. Reporting also helps strengthen communication, awareness, teamwork, and our overall culture of safety both on the water and around campus.
